In short
- PDF: Convert PDF to Google Form (also: turn PDF into Google Form, import PDF into Google Form).
- Word: Convert Word to Google Form.
- Google Doc: Convert Google Doc to Google Form.
- Scan or photo: Scanned PDF to Google Form or Image to Google Form.
- No file: Describe a Google Form from a short prompt.
Convert PDF to Google Form
That is still the most common search on this site. You have a questionnaire, job application, registration packet, or quiz as a PDF and need a Google Form in Drive, not a retyping project.
The PDF to Google Form guide covers what a readable PDF looks like, how Doc2Form maps multiple choice and short answer fields, and what happens when tables or two-column layouts show up. If your file is a survey with rating scales, also see Survey PDF to Google Form. For tests and exams, see Quiz to Google Form.
Turn Word and Google Docs into Google Forms
Many teams draft in Word first. The Word to Google Form guide walks through .docx uploads: headings, numbered lists, and option lists that survive export.
If the content still lives in Google Docs, use Google Doc to Google Form for shared drafts, comment-heavy templates, and handouts you want as an online Form without copy-paste.
Convert documents when the format is mixed
Not every upload is a single clean PDF. HR packets, onboarding checklists, and multi-section intakes often mix question types on several pages. The Document to Google Form guide is the umbrella page for PDF, Word, and describe mode when you care about the outcome (a Form plus Sheets responses) more than the file extension.
Scanned PDFs, phone photos, and images
Paper still shows up. Phone photos of worksheets, scanner PDFs, and PNG screenshots need OCR and layout guessing before any converter can build a Form.
Start with Scanned PDF to Google Form for PDF exports from a copier or scan app. For JPG/PNG uploads and screenshots, see Image to Google Form. Both pages spell out lighting, skew, and when to retake the photo instead of fighting the upload.
Describe a Google Form without a file
When you know the questions but do not have a document yet, describe mode is faster than opening a blank Form. The Describe a Google Form guide shows briefs that work well (audience, length, anonymous or not) and what to edit after the first draft lands in Drive.
What every guide includes
Each page below is written for a specific search intent, but they share the same job: extract structure, create a native Google Form, link it to Sheets.
- Quality in, quality out. Blurry scans and dense tables convert worse on every tool. The scan and PDF guides call out realistic limits before you upload.
- Drive-native output. The Form lives in your Google account. Share it like any other Form; responses go to Sheets.
- Honest scope. Doc2Form does not host forms on a third-party URL or run Stripe on the Form. If you need that, start with Comparisons instead.
