Your evaluation pack should work for every cohort
You run the same programs with new groups, but your evaluation form is already settled - wording your SMEs approved, scales that match the report template. The slow part is not facilitation; it is rebuilding that instrument in Google Forms whenever someone asks for a digital version or sends a revised PDF. Copy-paste across twenty-five items drifts. Starting from a blank Form burns time you planned for debriefs and follow-ups.
Doc2Form reads PDFs and Word files up to 5 MB and builds a matching Google Form in your Drive. No file? Use describe mode and type a short brief - for example a post-training survey with ratings and comment boxes - then edit what comes back. You keep control: adjust sections, mark fields required, duplicate per location, and share links the way your org already does with Forms.
That workflow fits needs assessments before design, course feedback during rollout, knowledge checks after modules, and certification-style item sets - the documents repeat; the audiences change.
Track scores and themes in Sheets over time
From the form's Responses tab, send answers to Google Sheets. Each row gets a timestamp, so you can compare cohorts, watch averages on scored items, and scan open text for recurring themes. When leadership asks how the last six deliveries compared, you are looking at the same questions each time - not a series of hand-typed variants that never quite matched.
For items with clear correct answers, quiz mode behaves like any Google Form you built by hand: assign points, auto-grade multiple-choice and checkbox questions, and export results alongside facilitator notes. Quality follows the source: clean Word outlines convert predictably; messy scans need more cleanup before they are trustworthy.
Built for teams on Google Workspace
Forms land in Drive under your account, with sharing and ownership rules your company already uses. Trainers in different regions can duplicate a master form, or you can transfer ownership when programs hand off - standard Google workflows, no separate form host to onboard.
Common questions
Does quiz grading work with Google Forms quiz mode?
Yes. Doc2Form creates a normal Google Form. Open the quiz settings, add point values, choose correct answers for objective items, and let Google score submissions. You still review the draft after conversion - especially for nuanced wording - before you publish.
What about Likert scales and rating grids?
When your PDF or Word file shows clear rows and labels, conversion usually maps them to multiple-choice, linear scale, or grid-style questions Google Forms supports. Odd layouts or merged cells may need manual cleanup. If the file is ambiguous, fix the structure in Word first for a cleaner pass.
Do scanned PDFs work?
Sometimes. Native PDFs and Word files give the best results. Scanned paper forms can work when text is sharp and typed; blurry photos, faint toner, or heavy handwriting reduce accuracy. If you struggle to read a page, expect the same limits from conversion.
What does it cost?
Your first form is free with no credit card. Credit packs follow for additional usage. The project is open source if you prefer to self-host without usage caps.
How do we share forms across locations and teams?
Each person signs in with their own Google account; forms save to their Drive like any other file. Add editors, duplicate a master form per site, or move ownership when a program owner changes. Workspace sharing rules apply - no shared login required for distributed L&D teams.
