In short: Turn registration, waiver, and volunteer PDFs into Google Forms for clubs and leagues. Not a full league-management platform. First form free.

Season registration should reuse last year's wording

Clubs keep waivers and medical contacts in Word. Coaches rebuild Forms each season and invent new option lists.

Import the master packet, duplicate per age group, and adjust only team names. Describe mode covers a rainout RSVP.

Based on a sample of 1,775 Google Forms generated by Doc2Form users, containing 55,761 questions across 47 languages. That is an average of 31 questions per form - roughly 21 minutes saved per form versus rebuilding each question by hand in the Forms editor (assuming ~40 seconds per question).

Waivers and medical limits

Doc2Form does not provide legal advice on waiver enforceability. Not TeamSnap or SportsEngine. Keep emergency medical paths per your association rules.

A real example

A youth soccer registration PDF with jersey size and medical contacts becomes a Form; the registrar exports Sheets for jersey orders.

Common questions

Can we collect registration fees through the Form?

Google Forms does not have a native "Add to Cart" or secure payment processing feature. We recommend linking to your payment processor (like PayPal, Stripe, or Venmo) in the "Confirmation Message" after the parent submits their info, or embedding the Form on a page that handles the transaction first.

Does this replace our primary registration system like SportsEngine or TeamSnap?

No. National bodies often require registration in their specific portals for insurance and roster card generation.

Can we use "Describe mode" for tournament RSVPs or pizza parties?

Yes. If you are running a one-off weekend event and do not have a PDF yet, you can type "RSVP for 3v3 basketball tournament with team name, division, and coach cell phone" into Describe mode to generate a quick signup link you can text to your group.

What about photo releases and social media permissions?

You can include your standard photo release language in the Form. Once imported, you can add a required "Yes/No" question for "I consent to photos being used on the club's social media" to track permissions per player.
