Exact wording and consistent scales matter. When your only copy is a PDF or Word template, rebuilding in Google Forms means long copy-paste sessions and a real risk of wrong anchors or missing items.

Doc2Form reads PDF or Word (up to 5 MB) and builds a standard Google Form: sections, choices, short answers, and linear scales when the source is clear. You still check against your protocol - but you skip the empty-canvas step each time you run a wave of data collection. No file? Describe mode accepts a short brief (for example, a gated screening form) so you can draft and refine.

Responses where your analysis already lives

Forms live in your Drive and link to Google Sheets in one click - handy if you already clean data in Sheets, R, or Python. Add study IDs or merge exports on your side as usual. Collaborators get editor access like any Google file so you keep one live form instead of parallel Word drafts.

Does this satisfy IRB or ethics requirements for sensitive data?

Google Forms and Workspace are general-purpose; they are not a certified system for every kind of regulated or highly sensitive research data. Your IRB or data steward should rule on identifiers, populations, and storage. Doc2Form only helps you assemble the form.

Can it reproduce Likert and matrix-style scales?

It maps what reads clearly: linear scales, multiple choice, checkboxes. Dense matrix tables in a scan or PDF can be fuzzy. Compare the draft to your validated PDF and fix endpoints and required fields in Forms before launch.

Will my validated instrument stay validated after conversion?

Treat the output as a draft. Licensed or fixed-order instruments need a line-by-line proof against the official version. Doc2Form cuts retyping time; it does not replace validation or claim psychometric equivalence.

What does it cost?

The first form is free with no credit card. Credit packs are planned for heavier use. The codebase is open source if you want to self-host.

Can my lab share one account or split work?

Each user signs in with their own Google account; forms land in that person's Drive. Use shared Drives or add collaborators on the form - same as any Google Form. Doc2Form does not provide a shared lab login.
