Your feedback templates are already on paper or in PDF
CX teams rarely start from a blank page. CSAT and NPS documents live in shared drives, complaint flows sit in Word, and support intake PDFs predate your current tooling. Copying them into Google Forms by hand is slow and easy to get wrong - a missing scale point or a duplicated question undermines the score you report upstream.
Doc2Form closes that gap. Upload a PDF or Word file up to 5 MB and it maps questions, options, and sections into a standard Google Form. No file? Use describe mode: type a short brief (for example, anonymous 10-question NPS with one open follow-up) and iterate from the draft. Quality in, quality out: crisp Word exports or text-based PDFs convert more reliably than fuzzy scans or heavily designed pages where questions hide in graphics.
Track scores and comments in Sheets
Link the form to Google Sheets and responses land in one table you can filter, pivot, and share with ops or analytics. That is the natural home for CSAT averages, NPS buckets, and verbatim trends without exporting from a separate survey product. Add a column for follow-up owner, tag escalations, or chart week-over-week movement from the same Sheet your team already opens. For recurring pulses, duplicate the form or archive Sheet tabs by period - the workflow matches how CX teams already report in Workspace.
Fits your Google Workspace setup
Every form Doc2Form creates is a normal Google Form in your Drive. It follows your org's sign-in, sharing, and retention rules - important when customer data is involved. Collaborate with enablement or product on copy, or keep ownership with CX; permissions work like any other Doc. If respondents should not need a Google account, use the link setting for anyone with the link. For internal-only pulse surveys, restrict to your organization and post the URL in Slack or your intranet. Customers get a familiar Google Forms experience on desktop and mobile when you share the public link.
Common questions
How do I track responses over time?
Connect the form to Google Sheets from the Responses tab. New submissions append as rows. You can add charts, filters, and duplicate the Sheet for each campaign, or use one Sheet with a date column for trend views.
Will customers have a decent experience on phones?
Google Forms is responsive by default. Short surveys with clear scales and limited typing work well on mobile - keep labels concise after conversion so the form stays easy to tap through on a small screen.
Which question types come through?
Typical CSAT and NPS patterns map well: multiple choice, linear scales, checkboxes, short and long text, and sections. Very complex branching may need a quick pass in the form editor after import. Clean source documents produce cleaner structure.
What does it cost?
Your first form is free with no card required. Paid credit packs are available for ongoing use. The project is open source if you prefer to self-host without usage limits.
Can I share the form with external customers?
Yes. Google Forms supports links for anyone with the link, or restricted to your organization. Doc2Form creates the form in your Drive; you choose whether respondents must sign in and whether the form is public. Test the link in an incognito window before you broadcast it.
