In short: Convert enrollment, permission, and family-survey PDFs into Google Forms for schools and districts. Classroom quizzes live on the teachers page. First form free; up to 5 MB.

District packets should not stall on midnight retyping

Enrollment and permission masters are refined by legal. Schools still retype them into Google Forms and drift from the approved wording.

Doc2Form imports the digital PDF or Word packet into a Form in your Workspace for Education account. Families finish on phones; you keep SIS as the record. For classroom quizzes, use the teachers role page and Quiz to Google Form.

Based on a sample of 1,775 Google Forms generated by Doc2Form users, containing 55,761 questions across 47 languages. That is an average of 31 questions per form - roughly 21 minutes saved per form versus rebuilding each question by hand in the Forms editor (assuming ~40 seconds per question).

FERPA, COPPA, and SIS boundaries

Configure Workspace for your privacy rules. Doc2Form is not PowerSchool or Canvas. It does not certify FERPA compliance for you - your district policies do. Messy scans may need cleanup.

A real example

A field-trip permission PDF with nurse meds and media release becomes a Form; nurses get a Sheet export instead of backpack paperwork.

Common questions

Is this the same as the teachers page?

No. This page is for school and district packets - enrollment, permissions, family surveys.

Does this replace our primary SIS or LMS like PowerSchool, Canvas, or Google Classroom?

No. Doc2Form is a productivity bridge for the "capture" and "assignment" phases.

Can parents upload photos of physician forms or student IDs through the Form?

Yes. Once the Form is generated in Google Forms, you can enable "File Upload" questions.

How do we handle math and science notation in PDF imports?

Images of equations and diagrams import when they are embedded as images in the source PDF. For very dense LaTeX or complex scientific notation, we recommend a quick pass in the Google Forms editor to ensure formatting is preserved.
