A yoga waiver is the record a studio keeps when a guest says they understand that class involves physical effort and a chance of strain or injury. Does this waiver replace a lawyer?

No. It records that a guest shared contact details and typed their name under your wording. Have a lawyer review the text if you need a release that will hold up for your studio.

Can guests draw a signature?

This question asks them to type their full legal name. After the form is in your Drive you can add other fields, but a typed name plus the date is what this template stores.

Should I ask about injuries?

The source form does not. Add a short question if your teachers need to know about a recent injury or pregnancy before class.

Who should fill it out?

The person taking class. If a minor is practicing, add a parent name and signature rather than treating the child's typed name as consent.
