About this template
Front desks still take half of their bookings in email threads: room type in one message, dates in the next, a flight number at midnight. This template puts that on one form. The guest enters a name and email, picks a room type, gives the party size, an arrival date and time, and a departure date. They say whether they want the free pickup, add a flight number if they do, and leave special requests.
The source title says payment, but the questions do not collect a card number, and this template does not add one. Taking cards in a Google Form is a bad idea. Use your property system or a payment link, and keep this form for the reservation details. The original submit button is not a question, so it is not included.
A small inn can send the link from the website. A larger property can use it for the pickup list even when the room is already sold in another system. Change the room list to the types you actually have before you share it.

FAQ
Why is there no card field?
The source questions never asked for a card, and a Google Form is the wrong place to store one. Send a payment link from the system you already use.
Can I change the room types?
Yes. Replace standard, deluxe, suite, and accessible with the names on your rate sheet after you save the form.
What if we do not offer pickup?
Delete the pickup and flight questions. They are only there because the source form had them.
Does the guest get a confirmation?
Google Forms can email the respondent a copy of their answers. Your reservation is still confirmed when you accept it, not when the form is submitted.
Can one form book more than one room?
This version is one stay. For a group, either duplicate the room questions or collect a room count in the special requests until you rebuild it.
