This checklist is the long source list, turned into the same three-way mark for every condition line: OK, needs repair, or not in this room. It starts with who inspected and the room number, then the door, the lock, the exit sign, the walls, the furniture, the bed, the windows, and the bathroom down to the caulk, the toilet, and the fan. Closet, safe, iron, TV, phone, air conditioning, coffee pot, fridge, and microwave are on it too.
TV make and serial number stay as text, because those are identifiers, not a pass or fail. A room note at the end is for anything the list missed. An image-slider field from the source is left off. Both copies are kept so you can delete the extra once you are in the form.

A villa without a microwave should not make staff mark a microwave every time, beyond the not-in-this-room option.

Why are there so many questions?
The source checklist walked the room item by item. Delete lines your property does not have. Not in this room is there for the ones you only sometimes have.
Can I attach a photo of the damage?
Not in this file. Add a file-upload question in the Forms editor on the items where a photo helps maintenance.
What are the duplicate lines?
The source listed one sink drain and one air-conditioner cover twice. Both are included. Remove the copy you do not want.
Who fills in the TV serial number?
Whoever is inspecting, if they can read it. The field is optional so a missing TV does not block the rest of the list.
How do I find the repairs later?
