About this template
A placement test is how a school decides which class a new student should join. This template is a short English example, not a finished exam. Ten items are grammar blanks (comparatives, vocabulary, prepositions, a wish, a relative clause, and a few others). Then there is a writing sample and a score box the teacher fills in later.
The sentences are the ones that came with the source form, with the HTML cleaned up so students see a normal blank instead of a code. They are a starting point. Replace them with items you are allowed to use, at the level you actually teach, before you send the link. A Google Form will not grade the blanks for you. The score field is optional and meant for the teacher after review.
Language schools and college writing programs can turn responses into a Sheet and sort by the score column. Add a student name and email if you need to know who wrote which sample. Those fields were not on the source, so they are not invented here.

FAQ
Will Google Forms score the blanks?
No. Students type the missing words. You read them and put the result in the score box, or you add an answer key later in the Forms editor.
Can I use these sentences as-is?
Only if you are happy to teach from them. They are a sample. Swap in items you wrote or have permission to use.
Where is the student's name?
The source test did not ask for one. Add a name and email at the top if you need to match a score to a person.
What form type is this?
It is an assessment of the student's English, so the page is filed under Feedback & Evaluation, not as a poll.
Can I add listening or speaking?
Not in this template. A Form can link out to an audio file, but speaking still needs a meeting or a recording you set up yourself.
