About this template
Airlines, border officers, and hotels sometimes ask who a child is traveling with when the adult is not a parent. This template is a written permission slip for a trip with a grandparent. It records both parents' names, phones, and addresses, the child's name, date of birth, age, and ID type, then the grandparents, the travel dates, and a short trip note. Each parent types a signature and the date.
It is not a government form, and it does not replace a passport, a visa, or a letter your destination specifically requires. Families use it so the details are in one place before anyone gets to the airport. A school, a tour operator, or a family lawyer can send the same link and keep the submission with the booking.
Fill it while both parents can still answer. If only one parent has legal custody, say that in the trip details instead of leaving the other signature blank without an explanation. Change the ID list if your country uses a different document.

Is this an official government form?
No. It is a family permission record. Check the airline and the destination for any letter, notarization, or form they require on their own.
What if only one parent is signing?
Use the travel details field to say why, for example sole custody. Do not invent a second signature.
Which ID should we list?
The document the child will carry on this trip. Passport, birth certificate, and national ID are the starting options. Add another if you need it.
Can grandparents fill this out themselves?
The useful signature is the parent's. Grandparents can help type the trip dates, but the parent fields should be completed by the parents.
How long should we keep the response?
At least through the trip. Your own retention rules apply after that. The response lives in your Google Form.
